1. This online application must be completed by the applicant only.
2. Be sure to answer each question. Questions that are unanswered or that have incomplete answers may disqualify applicant. This is particularly true of the questions about work history, driver’s licenses, license suspension, tickets and accidents.

4. Applicant is aware that inquiries will be made to all prior employers and leasees for the purpose of investigating the applicant’s background. DOT rule 391.21(b)(11) and 391.23.
5. Applicant is aware that a urine sample will be collected and tested for the presence of controlled substances in accordance with FMCSR 382.301.
6. If hired, applicant will be required by the terms of a labor agreement between Active Truck Transport LLC, Active USA Inc. & Active Canada and the International Brotherhood of Teamsters (I.B.T.) that requires employees to either join the I.B.T. or in lieu of joining, pay an amount of money equal to that paid by other employees for the representational activities of the I.B.T. performed in conjunction with the applicable labor agreement.
